Job SummaryJob description/Responsibilities: PART-TIME (0.6) AM SHIFT APPROXIMATE PAY RANGE: $29.17 - $39.20 JOB SUMMARY: The Skilled Nursing Home (SNF) Health Information Management (HIM) Specialist is responsible for maintaining the residents? health records during and after SNF stays. This position ensures compliance of legal medical record through concurrent and retrospective audit of medical charts. The HIM Specialist reviews medical documentation to verify the completeness of the documentation, and assign any deficiencies to the responsible medical staff member for completion. The HIM Specialist completes release of information for SNF medical record requests. The HIM Specialist provides customer service to family members and visitors for HIM related queries. The HIM Specialist will assist HIM Department by performing additional duties as assigned by the HIM Director. RESPONSIBILITIES: 1. Maintain confidentiality, protecting patient information at all times: minimum information necessary to those with right and need to know. 2. Verifies receipt of all records and promptly locates records not received. Audits charts in real time. 3. Accurate preparation of medical record folders according to department procedures. 4. Assists in resolving medical record discrepancies when identified, and communicate corrections in medical record number to appropriate personnel. 5. Helps with purging of medical records as needed. 6. Accurately maintains and consistently utilizes the chart location system. 7. Receives, verifies and distributes all transcribed reports appropriately. 8. Audit and correct demographic errors (patient name, DOB, MRN and account number) identified on transcribed reports. Notify transcription services of need for correction to appropriate work-type. 9. Determine patient identification of loose reports and accurately file in the appropriate location (according to established chart order) within the correct visit of patient?s medical record. 10. In an accurate and timely manner assemble the discharged records into the legal medical record per established chart order and verify each document belongs to that record. 11. Performs all other duties as assigned or required.Required Qualification: EDUCATION, EXPERIENCE, TRAINING 1. Minimum two years experience in medical records management required. 2. Associate degree or equivalent required. 3. Experience with SNF medical records required. 4. Understanding of federal and state SNF regulations as it relates to HIM. 5. Completion of course in Medical Terminology or Health Information Management preferred. 6.
