Make a Real Difference — In Patients' HomesBasin Home Health & Hospice is looking for a licensed Physical Therapy Assistant (PTA) to join our home health team on a PRN or part-time basis. This is a pay-per-visit role built for a PTA who wants flexibility, meaningful one-on-one patient relationships, and the kind of clinical autonomy you don't get in a clinic or hospital setting.You'll work directly with patients across the Farmington area, helping them regain strength, mobility, and independence at home — collaborating with a supervising Physical Therapist and an experienced interdisciplinary care team that has served the Four Corners region for years.What You'll DoProvide skilled physical therapy treatments in patients' homes under PT supervisionImplement care plans and therapeutic interventions as directedMonitor and document patient progress accurately and on timeEducate patients and families on home exercise programs, safety, and mobilityAssess home environments for safety and care appropriatenessParticipate in biweekly interdisciplinary episode management meetingsComplete documentation in compliance with agency and HIPAA standardsWhat We're Looking ForRequired:Current New Mexico PTA license in good standingAbility to work independently in a home-based care settingComfort navigating rural and semi-rural patient environmentsStrong verbal and written communication skillsProficiency with EMR documentationSchedule & CompensationPrimarily Monday–Friday, with flexible daily scheduling based on patient visitsPay-per-visit structureCompany vehicles or mileage reimbursement provided401(k) availableWhy Basin?Basin Home Health & Hospice is locally owned and has been part of the Four Corners community for over 30 years.
