Job DescriptionStart Date: 07-14-2025 End Date: 10-10-2025 Provider Type: Advanced Practice Coverage Type: Scheduled Clinical Hours Only EMR System: Epic Practice Setting : Outpatient Patients Per Shift: 20 Does your Facility require physicians to be board certified?: Yes Fellowship: NA Minimum Board Certification: Certified Estimated Credentialing Timeframe (Days): 60-90 What is the license requirement to bid?: Licensed in CA only Chart closure within 72 hours Schedule = 18 PPD, minimum is 18 PPD Monday-Friday 8-5 Mix of peds/adults A decision is typically made within 7-14 days APC's with 3 years of experience in similar setting with panel management experience. PAs must have CA Controlled Substance Education Course Certificate NPs must have Furnishing License with Schedules II Active DEA needed before start Credentialing takes 30-90 days The following immunizations are required for credentialing (before start date): 2022-2023 Flu Vaccine, TB Status/X-Ray Results (results must be within the 90 days before start date), Positive MMR Titer or Complete Immunization Series, Positive Hep B Titer or Complete Immunization Series, Positive Varicella Zoster Titer or Complete Immunization Series, Tdap Immunization (must be within the last 10 years).
