Under the direction of the Assistant Regional Director and/or Director, the Care Coordination Program Assistant is responsible for uploading documents into the Electronic Health Records (EHR) system and maintaining the physical office locations, including office supplies, mail, scheduling meeting rooms, and reporting any maintenance requests.Essential Job Functions:Upload all required documents including but not limited to enrollment documents, disenrollment documents, and annual review paperwork into PCC's EMR.Serves as the main contact for the region and communicates all CAS assessment information to Care Managers from the IT representative which includes but is not limited to monitoring status of completion.Schedules meetings as needed, including coordinating the use of conference and/or meeting areas.Responsible for maintaining regional Care Coordination staff roster and contact informationEnsure completion of documents; filing pertinent to department's needs including but not limited to documents needed for internal and external audits.Maintain regional fax folders and distribute documents accordinglyResponsible for creating new enrollment folders and mailing as neededComplete non-billable notes as needed to document completed tasksDevelops and ensures consistency for regional correspondence as needed to support the Care Coordination team (e.g. create, copy, and distribute letters, memos, reports, and forms on a timely basis).Maintain the highest level of confidentiality, following all HIPAA policy and procedures.Meet professional obligations through efficient work habits such as meeting deadlines, honoring schedules, coordinating resources and meetings in an effective and timely manner.Perform all other duties relevant to the position as requested.Knowledge, Skills, and AbilitiesProficient in the use of all Microsoft applications (excel, work, power point, Microsoft outlook and access).Ability to work independently under minimal supervision.Ability to communicate effectively, both orally and in writing.Ability to be efficient and productive in a fast-paced environment.Must be detail oriented and have the ability to multi-task.Education and Experience:Associate's degree preferred, HS/GED diploma.One year minimum administrative assistant experience requiredPhysical Requirements/Working Conditions:Ability to sit continuouslyAbility to reach above shoulder levelAbility to turn/twist upper bodyAbility to use hands for repetitive action and fine manipulating for the purpose of keyboardingThe listed salary range represents the organization's good faith and reasonable estimate of the range of possible compensation at the time of posting.
