This role emphasizes preventive medicine, chronic diseasemanagement, and building meaningful, long-term patient relationships in asupportive outpatient environment. You will work within a collaborative, multidisciplinary care modeldesigned to improve patient outcomes, enhance access to care, and reduceunnecessary hospitalizations. Our approach prioritizes personalized care plans,proactive health management, and patient education. Key ResponsibilitiesProvide comprehensive primarycare services, including evaluation, diagnosis, and treatment of acute andchronic conditions.Deliver preventive care, wellnessvisits, and health screenings with a focus on early intervention.Develop and manage individualizedcare plans for a predominantly geriatricclosely with nursepractitioners, care coordinators, and support staff to ensure seamless,patient-centered care.Utilize electronic health records(EHR) to document clinical encounters and track patient outcomes.Engage in quality improvementinitiatives and contribute to a culture focused on continuous careenhancement.Educate patients and families ondisease prevention, treatment plans, and healthy lifestyle choices.
