Responsibilities:Assess patients' physical, emotional, and cognitive conditions to develop personalized treatment strategies.Implement therapy plans to improve patients' daily living skills and physical functions.Document patient progress accurately and communicate with the healthcare team.Adapt therapy sessions based on patient response and evolving needs.Educate patients and caregivers on treatment goals and home exercise programs. Qualifications:Hold a valid Occupational Therapist license and state registration.Possess proven experience in a skilled nursing or long-term care environment.Demonstrate strong assessment and clinical reasoning skills.Exhibit excellent communication and team collaboration abilities.Be adaptable to a dynamic clinical environment.
