In Office LVN Case Manager
Job Overview
We are seeking a dedicated and compassionate Case Manager to join our healthcare team. The ideal candidate will have experience in a Level II Trauma Center and possess a strong background in medical-surgical, emergency medicine, and behavioral health. This role is essential in coordinating patient care, ensuring effective discharge planning, and providing support for both inpatient and outpatient services. The Case Manager will work closely with patients, families, and healthcare providers to facilitate optimal care and recovery.
Duties
Assess patient needs through comprehensive evaluations and interviews.
Develop individualized care plans that address the medical, emotional, and social needs of patients.
Coordinate with multidisciplinary teams to ensure seamless transitions of care.
Facilitate discharge planning by preparing patients for a safe return home or transfer to other facilities.
Monitor patient progress and adjust care plans as necessary to meet evolving needs.
Advocate for patients' rights and ensure they have access to necessary resources and services.
Maintain accurate documentation of case management activities in compliance with regulatory standards.
Educate patients and families about available services, treatment options, and community resources.
Collaborate with healthcare providers to enhance the quality of care delivered.
Qualifications
Bachelor’s degree in nursing or social work; Master’s degree preferred.
Previous experience in case management within a healthcare setting is highly desirable.
Familiarity with medical-surgical practices, emergency medicine protocols, and behavioral health issues.
Knowledge of discharge planning processes and geriatric care considerations.
Strong communication skills, both verbal and written, with the ability to engage effectively with diverse populations.
Excellent organizational skills with attention to detail in documentation and reporting.
Ability to work collaboratively within a multidisciplinary team environment.
Certification in case management (CCM) or similar credential is a plus.
