Physical Therapist in Mineral Wells, WV
Deliver skilled physical therapy services in patients' homes across a multi-county area in West Virginia, focusing on orthopedic conditions, generalized weakness, and cardiac rehab. This contract role offers self-scheduling flexibility and the opportunity to work closely with an experienced rehab team using Epic documentation.Key Responsibilities:Provide 5-6 patient visits per day, including Starts of Care, recertifications, discharges, and regular visits.Conduct thorough physical therapy evaluations and treatments for home health patients presenting with general orthopedic issues (hips, knees), weakness, and cardiac rehabilitation needs.Document care thoroughly and accurately using the Epic electronic health records system.Collaborate with PT management and rehab aides to optimize patient outcomes.Adapt schedule and patient assignments flexibly to accommodate geographic areas needing coverage, focusing ideally on Webster and Nicholas counties.Qualifications & Desired Experience:Current physical therapy license valid for practice in West Virginia.Basic Life Support (BLS) certification.Experience or comfort treating a variety of home health patients with orthopedic and cardiac diagnoses.Willingness to learn and utilize the Epic documentation system.Positive, adaptable, and self-directed approach to care and scheduling.Location & Community: Based in Mineral Wells, WV, this role serves diverse rural counties including Braxton, Gilmer, Calhoun, Nicholas, Webster, Clay, Randolph, and Pocahontas.
