This role partners closely with the Center Director and ADT Supervisor/Assistant Director to support consumer training, documentation, intake, and administrative operations. The Case Manager plays a vital role in maintaining accurate records, coordinating with families and community partners, and delivering high-quality, person-centered services that align with our mission.\n\nWhat we offer\nLive and on-demand professional development\nMedical, dental, and vision coverage\nPaid holidays, vacation, sick, and personal time\nEmployee Assistance Program\nSupplemental insurance options\n403B retirement savings plan\nEasterseals Cares Wellness program\nGenuine work-life balance\nThe chance to make a lasting impact in your community\n\nA snapshot of what you'll be doing\nMaintain complete and compliant consumer files, documentation, service authorizations, and electronic records\nCoordinate intake, scheduling, and implementation of Individual Program Plans (IPPs) in collaboration with families, support coordinators, and community agencies\nMonitor attendance, progress reports, and skill development to support individual goals and program quality\nEnsure compliance with all regulatory, contract, funding, and accreditation requirements\nCollaborate with Center leadership to support consumer training, orientations, quality assurance, and regulatory reviews\nAssist with administrative operations including billing documentation, data entry, tours, and general office support\nParticipate in community outreach and marketing efforts as needed\nMaintain required agency, regulatory, CPR, and First Aid trainings\n\nQualifications\nAssociate's degree (A.A.) or equivalent from a two-year college/technical school; or one year of related experience/training; or equivalent combination of education and experience\nCPR and First Aid Certification (or ability to obtain)\nCompletion of required APD mandatory trainings\n1–3 years of relevant experience in case management, adult day services, or elder services\nKnowledge of community-based resources for older adults and caregivers\nStrong communication, organizational, and problem-solving skills\nProficiency in Microsoft Office, Adobe Acrobat, and internet-based tools\nAn energetic, resourceful and professional individual who will promote a positive, respectful and team-based environment\nAbility to meet our approved driver qualifications in order to drive your personal vehicle, as required, for program-related needs (minimum age of 21 and good driving history)\n\nEasterseals Florida has been rated a Great Place to Work (2020 – 2025) by our employees!
