Physical Therapist - Full Time

Large Growing Certified Home Health Agency (CHHA) is seeking an experienced Physical Therapist to see patients in Suffolk County only! Will break up county by zip codes. Excellent opportunity for PT with outpatient or rehab experience seeking more autonomy and flexibility. Responsibilities: Evaluate Home Care recipients and develop an appropriate physical rehabilitation plan. Help Home Care recipients through their tasks and monitor their performance. Provide comprehensive assessment and treatment for patients in home care setting Assess need for and teach each Home Care recipients how to use Durable Medical Equipment Educate Home Care recipients on what to anticipate in different stages of their recovery. Maintains up-to-date/accurate documentation for assigned cases Complies with federal, state, and local legal and professional requirements. Requirements: Graduate of an accredited Physical Therapy Program Valid NYS Physical Therapist license Reliable transportation required Willing to travel to patient's homes Job Type: Full-time Pay: $110,000.00 - $115,000.00 per year Medical Specialty: Home Health Schedule: 8 hour shift Work Location: On the road
