Occupational Therapist - (Per Diem)
Job Overview
Ultimate Mobility is seeking a dedicated and compassionate Occupational Therapist (OT) to join our healthcare team on a per diem basis. In this role, you will provide patient-centered care across inpatient, outpatient, and home health settings, helping patients regain independence and improve their ability to perform daily activities. You will develop and implement individualized treatment plans that address physical, cognitive, and sensory needs, while collaborating with a multidisciplinary team to achieve optimal outcomes.
Key Duties & Responsibilities
Conduct comprehensive occupational therapy evaluations, including assessments of functional abilities, self-care, fine motor skills, cognitive and sensory processing, and environmental needs
Develop, implement, and modify individualized treatment plans to improve patient function and independence
Provide therapeutic interventions, such as ADL (activities of daily living) training, adaptive equipment training, splinting, fine motor exercises, and cognitive or sensory integration activities
Educate patients and caregivers on adaptive strategies, home programs, and safety techniques
Collaborate with physicians, nurses, therapists, and case managers to coordinate care and support discharge planning
Monitor and document patient progress accurately using EMR systems
Maintain compliance with professional, ethical, and legal standards of occupational therapy practice
Stay current with advancements in occupational therapy across specialties such as pediatrics, geriatrics, neurorehabilitation, and mental health
Adhere to all safety, infection control, and facility policies
Requirements & Qualifications
Valid and active Occupational Therapist license in the applicable state/region
Master’s or Doctorate in Occupational Therapy from an accredited program
Completion of the NBCOT exam or state equivalent
Current CPR/BLS certification
Proof of active professional liability (malpractice) insurance, maintained independently by the OT
Experience in hospital, outpatient, or home health care preferred
Strong knowledge of anatomy, physiology, kinesiology, therapeutic techniques, and adaptive equipment
Excellent communication, organizational, and patient care skills
Ability to work independently across multiple care environments
Ability to meet the physical demands of the role, including assisting patients as needed
