The Advanced Clinical Pharmacist - Critical Care will play a crucial role in ensuring the safe, effective, and cost-efficient use of medications for patients in the ICU. This individual will uphold the highest standards of professional ethics and act as a patient advocate within a multidisciplinary rounding team.ResponsibilitiesParticipate in facility committees to establish and implement medication-related policies and procedures within the ICUDirect the critical care service line within the hospital, taking a lead role in planning and implementationIdentify opportunities to minimize drug costs through appropriate use and cost-containment measuresEducate nursing, physician, and ancillary teams on clinical pharmacy services, new drugs, or updated clinical guidelinesInterpret, transcribe, and verify medication orders accurately and efficientlyCompound and dispense pharmaceuticals accuratelyMonitor drug therapy regimens for contraindications, interactions, allergies, and appropriateness of drug and doseSustain the hospital formulary, minimize nonformulary procurements, and promote rational drug therapy selectionProvide accurate and timely medication information to healthcare staff and patientsAssist with clinical services such as drug consults, nutrition support, pharmacokinetics, anticoagulant monitoring, pain management, IV to PO, renal dosing, and other programs as defined by Pharmacy LeadershipDocument all clinical activities and interventions accurately and completelyComplete notification reports regarding medication errors and adverse drug reactionsSupervise pharmacy operation activities effectively and provide support to the entire pharmacy team QualificationsEducation: BS or Masters in Pharmacy from an accredited institution. Graduation from a college of Pharmacy accredited by the American Council on Pharmacy Education. Completion of an accredited PGY-1 residency program (Required), PGY-2 residency program (Preferred)License/Certifications: Active pharmacist license in good standing with the appropriate State Board of PharmacyExperience: 3 years of acute care experience as a Pharmacist required, 5 years' inpatient hospital experience preferred. Minimum of 3 years' experience in Infectious Disease/Critical Care in addition to a residency in Infectious disease or Critical care Ideal CandidateCustomer service abilities, critical thinking skills, decisive judgment, and the ability to work with minimal supervision in a fast-paced environment.General Comments:Shift Time: Mixed - Rotate every other weekendSalary Range: $56.55 (1 yr) - $68.69 (17 yr MAX)Sign-On Bonus: $10,000Relocation: Case by Case
