The position works in collaboration with treatment team to assess needs of mental health clients and support them in achieving mental health goals.\nESSENTIAL DUTIES AND RESPONSIBILITIES\nThe duties listed below are intended only as illustrations of the various types of work that could be performed. The omission of specific statements of duties does not exclude them from the position if the work is similar, related or a logical assignment to this class.\nAssists assigned clients to identify and progress toward achievement of client chosen goal.\nAdvocates for clients in all areas of treatment.\nEmpowers clients in meeting their goals.\nFacilitates communication and cooperation among agencies.\nParticipates in client medication support; coordinates communication between doctor, client and pharmacy.\nProvides prompt intervention in the event of a crisis both to stop the crisis and to notify when indicated, persons and agencies necessary for the resolution of the crisis situation.\nProvides “on-the-spot” support that is both helpful to the clients and consistent with the philosophy of the program.\nCarries a caseload of from 20 – 40 clients depending on acuity of clients, program/contract requirements and referrals on record; varies from program to program and contract to contract.\nAttends to the safety, health, and wellbeing of clients.\nWrites Quarterly reports to provide necessary information to Regional Center point of contact for each client on caseload.\nReviews and ensures timely completion of Special Incident Reports, as well as Mandated Reports.\nComplete paperwork as assigned in a timely manner and meets agency standard for billable units per month; ensure that all notes meet billing standards for content and configuration.\nVisits clients in home periodically to provide direction and support\nDrives on agency business using personal vehicle; may be required to transport clients, equipment, protected health information (PHI), and/or confidential personnel or TPCP agency information.\nMINIMUM QUALIFICATIONS AND CERTIFICATIONS & LICENSES\nEducation, Training and Experience:\nA typical way of obtaining the knowledge, skills and abilities outlined above is through graduation from a four (4) year college or university with a bachelor’s degree in Social Work or related field and four (4) years varied experience as a provider of mental health services. Additional qualifying experience may be substituted for education.\nLicenses; Certificates; Special Requirements:\nCalifornia driver’s license & current vehicle insurance/registration if driving; and,\nReliable means of transportation capable of passing vehicle safety inspection if more than five years old excluding all modes of two-wheeled transport inclusive of bicycles, mopeds and motorcycles.\nSchedule: Monday – Friday, 8:30 am - 5:00 pm\nEducation\nRequired\nHigh School or better\nPreferred\nBachelors or better in Social Work or related field\nSkills\nPreferred\nSocial Services - Organization\nSocial Services - Resource Access\nSocial Services - Crisis Intervention\nSocial Services - Providing Mental Health Sup\nBehaviors\nPreferred\nTeam Player: Works well as a member of a group\nEqual Opportunity Employer\nThis employer is required to notify all applicants of their rights pursuant to federal employment laws.
