The Post-Acute and Long-Term Care (PALTC) Advanced Practitioner is an APRN or PA who independently manages patient care in PALTC settings, collaborating with facility staff to improve outcomes, enhance patient and family experiences, reduce hospitalizations, and control costs.Responsibilities include conducting physical exams, diagnosing and treating illnesses, managing chronic conditions, providing preventive care, and developing care plans. The AP works autonomously within scope, making clinical decisions and escalating when necessary. They build strong relationships with facility leadership and interdisciplinary teams, ensuring holistic care and effective communication with patients’ families and external providers.The role emphasizes compliance with regulations, accurate documentation, infection control, health education, and participation in quality initiatives. Candidates must be self-directed, possess excellent communication skills, be proficient with EHR and telemedicine tools, and be physically capable of responding quickly in emergencies. Required qualifications include relevant degrees, at least 2 years of experience (preferably in long-term care), and board certification.
