The Hospice nurse builds from the resources of the Interdisciplinary Group (IDG) to plan and direct services to meet the needs of individuals and families within their home environments. May provide patient care to premature infants, infants, pediatrics, adolescents, young, middle and older adults.\n\nPosition Specific Responsibilities\nCompletes comprehensive assessments of patients and families to determine hospice needs, including physical assessments and histories of current and previous illnesses.\n\nDevelops, initiates, and revises individualized Plans of Care in collaboration with the IDG, using health assessment data to determine nursing diagnoses and establish goals.\n\nProvides professional nursing care by utilizing the nursing process, implementing palliative interventions, administering prescribed medications and treatments, and counseling patients and families on health care needs.\n\nProvides health care instructions to patients and families as appropriate per assessment and plan.\n\nCoordinates with the Patient Care Manager and IDG to ensure continuity of care, safe environments, and support for the emotional needs of patients and caregivers.\n\nCompletes, maintains, and submits accurate and timely clinical documentation, including patient condition, care provided, and pain/symptom management outcomes.\n\nCommunicates patient needs, condition changes, and physician orders to the IDG and supervisors; attends IDG meetings to review and update Plans of Care.\n\nFulfills accepted case assignments, participates in on‑call duties, and maintains productivity standards per organizational guidelines.\n\nSupervises and delegates responsibilities to ancillary personnel as appropriate.\n\nSupports the orientation, education, and development of team members, and provides input on CHHA performance and assignments.\n\nAssumes responsibility for professional growth through continuing education, in‑service programs, and other activities to enhance job‑related skills and abilities.\n\nActively participates in performance improvement initiatives and adheres to the Corporate Integrity Plan.\n\nEnsures financial prudence in care delivery by coordinating medications, supplies, equipment, and other necessary services defined in the Plan of Care.\nEducation and/or Experience\nCalifornia (CA)\nRequired: Education sufficient to meet certification, licensure, or registration requirements.\nPreferred: 1 year of recent experience in hospice, palliative care, home health, acute care, and/or skilled nursing as a registered nurse.\nSkills\nMathematical Skills: Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percentage.\nLanguage Skills: Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations.\nAbility to write reports, business correspondence, and procedure manuals.\nAbility to effectively present information and respond to questions from leaders, team members, investors, and external parties. Strong written and verbal communications.\nAbility to understand, read, write, and speak English. Articulates and embraces integrated healthcare at home philosophy.\nOther Skills and Abilities: Excellent observation, verbal and written communication, problem solving and basic math skills.\nVisual acuity and hearing to perform required nursing skills.\nPreferred working knowledge with computer applications, i.e. Microsoft Word/e-mail.\n\nCertifications, Licenses, and Registrations\nRequired within 30 days of hire: National Provider BLS – American Heart Association (Vendor Managed)\nActive state RN license (Vendor Managed).\n\nState Specific Requirements\nOther\nRequired upon hire: Active Registered Nurse License (Vendor Managed)\n\nPhysical Demands and Work Environment: The demands of this role necessitate a team member to effectively perform essential functions.
