As the In-House Nurse Practitioner (NP) you will serve as the primary provider of internal medicine services within the Skilled Nursing Facility. Being the top clinician at a single facility, you have the ability to truly build relationships with your residents, their families, providers & facility staff.
Trio Health Partners team is dedicated to improving clinical outcomes and preventing re-hospitalizations for patients in the Long-Term care facility. Our NP team makes a significant impact by addressing issues before they become critical in our Skilled Nursing - Long Term Care building.

Requirements
Active, unencumbered Illinois Nurse Practitioner license
Master's degree in Nursing from an accredited program
Strong clinical skills in primary care and preventive medicine
Excellent communication and interpersonal skills. Effectively communicate with elderly and chronically ill patients and families.
Ability to work independently and collaboratively within a healthcare team
Ability to meet with a minimum of 10 patient visits per day - Bonusing even when hitting minimum required number of patient visits.
Geriatric or adult medicine specialty preferred. Previous nursing background within the Skilled Nursing, Long Term Care, Assisted Living, Hospice, Palliative care backgrounds. Comfort working within this setting will make you highly successful!
Must be able to effectively communicate with elderly and chronically ill patients and families.
Understanding of Medicare, Medicaid and Health Plan benefit structures beneficial, not required
Ability to multi-task & Excellent patient care skills
Our Nurse Practitioners engage in close collaboration with the interdisciplinary team and physicians, assuming the role of a daily clinical leader. You will supervise their day-to-day care in conjunction with the facility’s nursing staff, receiving guidance from the medical directors, and attending physicians. In your capacity as an In-House Nurse Practitioner (NP), you will administer various types of care, including but not limited to:
Evaluation of new-admission visits
Monthly stability visits for long-term residents
Perform comprehensive physical examinations of patients
Evaluations for patients exhibiting changes in condition
Prescribe all necessary medications and treatments
Develop and implement patient management policies and procedures
Facilitate referrals to other healthcare professionals and medical facilities
Communicate with attending physicians and/or specialists regarding patient care
Advanced Care Planning
Coordination of discharge visits
Be comfortable and proficient with documentation and CPT coding - preferred
Support the physician/patient relationship and ensure timely and adequate communication, documentation of assessment findings, recommendations, need for additional services, emergency services required if necessary and need for follow up and timelines for follow up to primary care provider (PCP) and health plan as required.
Make general recommendations to members intended to improve members' knowledge of their chronic condition(s), such as information concerning recommended testing.
Address and close identified gaps in care (disease-specific or preventive).
Recognize emergent or urgent situations requiring escalation and take appropriate action as specified in Trio’s policies, and as determined by reasonable professional judgment and ethical professional practice standards.
