Case Manager

Position Summary:The Case Manager is responsible for providing comprehensive case management services, conducting educational lectures, and supporting clinical staff with various tasks. This role involves maintaining accurate case records, coordinating access to necessary services, and assisting with aftercare planning and referrals. The Case Manager also actively participates in clinical reviews, staff meetings, and professional development activities.Essential Responsibilities:Conducts didactics/educational lecturesAdvises clients individually or in group on issues related to risk prevention, diagnostic screenings, and community resources that improve social determinants of health outcomesResponsible for establishing trusting relationships with patients and their families while providing general support and encouragement.Assist clients in determining the steps needed to take in order to gain access to treatment services and support the client with identifying and adequately addressing barriersProvide clinical services to screen, assess, and determine appropriate level of careAssess social needs of program participants and provide resources to the extent possible for the client to utilize when in the communityAssist clients in locating, joining, and finding means to participate in self-help/support groups such as NA, AA and Celebrate Recovery.Maintain case records in accordance with Harbor Light standardsCoordinate and facilitate access to healthcare, social services, housing, financial assistance, and other support systemsAssist with aftercare planning and referralsParticipates in the review of all clinical issues, activities and program policies and proceduresAttends all clinical staff meetings, and team conferencesParticipates in educational courses, seminars, workshops, current literature and in-service meetings to enhance treatment skillsEstablish and maintain professional and timely documentation in the electronic medical records system. The case manager is responsible for his/her own documentation and is expected to review, correct, and seek assistance to ensure compliance.Maintain a working knowledge of current trends and developments in substance use disorder and co-occurring disorders by reading relevant materials and participating in continuing education seminars, webinars and other in-service training when offered.Perform other duties as assignedQualifications:Education/Experience:A bachelor's degree in social work or an equivalent comparable human services field is requiredExperience in case management preferredExperience with mental health and/or substance abuse populations preferredSkills, Knowledge & Abilities:Ability to determine needs and to work with multi-problem individualsAbility to develop rapport, trust and confidence in client relationshipsMust possess good communication and teamwork skillsStrong organizational skillsTB Test and police clearance requiredAuthorization to work within The United StatesComputer Skills:Excellent computer skills requiredExperience using electronic medical records systems preferredCertificates and Licenses:Complete Safe From Harm training, and keep current as neededMust possess MCBAP credential (CADC, CADC-M, CAADC) or have an approved Development Plan within 30 days of hire dateValid Michigan Chauffeur License and approved by The Salvation Army MVR
