Through strategic partnerships with governmental agencies and innovative programs like Full Service Partnership (FSP), Wraparound (WRAP), PIER, we foster long-term skills and support systems, enhancing the well-being of individuals and families across all stages of life.\nPosition Details:\nFull-Time\nLocations: Van Nuys\nKey Responsibilities\nProvide case management services to FSP children and their families, including the identification and pursuit of resources necessary and appropriate to meet clients' needs.\nProvide in-home direct service to families as needed.\nProvide supportive assistance to the client in the assessment and determination of need.\nAssist therapists in obtaining outcome measures and other documents as assigned.\nAssist with securing adequate living arrangements, vocational activities, schooling, supportive mental health services.\nAttend outside agency meetings with community liaisons as assigned.\nMaintain client charts in a responsible manner and meet all documentation paperwork deadline requirements\nConduct phone screening and outreach services for potential FSP clients.\nProvide case management assessments and may complete aspects of administrative intakes and group intakes on new FSP clients as assigned.\nQualifications\nBachelor’s degree in related field (psychology, sociology, social work, special education or human services) required\nMinimum 2 years’ experience working with children and TAY-aged clients with mental illness in various settings.\nStrong social and communication skills, verbal and written to work with and within the multidisciplinary treatment team and ability to flex work schedule to accommodate client needs\nValid CA driver’s license with good driving record and auto insurance; must be willing to drive in the community when needed.\nBenefits\nHelp Group offers wonderful training opportunities, a supportive, professional work environment, and great benefits, including:\nInsurance plans for Medical, Dental, Vision, and Life Insurance - options for 100% employer sponsored plans\n403b Retirement Plan\n529 College Savings Plan\nEmployee Assistance Program\nFlexible Spending Account\nVacation and Sick Leave\nBenefit Hub - Employee Rewards and Incentives\nPet Insurance\nLearn more about our Intensive Family Based Services here:\nFSP\nPIER Early Psychosis (EP) Program\nWraparound Program\nwww.helpgroup.org\nThe salary range represents the low and high end of the salary someone in this role may earn as an employee of Help Group.
