Vital is committed to improving the quality of life for individuals in need, with a focus on compassionate, patient-centered care, while supporting clients and their families throughout their healthcare journey.Role DescriptionWe have openings for full time and part time Home Health Nurses, experienced in providing comprehensive nursing care to patients in their homes, including developing and following individualized care plans, administering medications and treatments, and performing wound care. Additional responsibilities include monitoring patient progress, documenting care provided, collaborating with healthcare teams, and educating patients and their families on health management and prevention practices.QualificationsBackground in Nursing and Medicine with a valid Illinois nursing licenseExcellent communication skills to provide education and coordinate care with patients and familiesAbility to work independently and manage time effectively in a home environmentCPR certification and knowledge of safety and infection control proceduresPrior experience in home healthcare is preferredFull-Time Salary starting at $90,000 NEW COMPANY CAR PROVIDED Including gas and insuranceBENEFITS: INSURANCE: Health/Dental/Vision/Life PTOPART TIME PRN RATES: Admit: $90.00 Recert/ROC $70.00 Routine Visit $60.00 Mileage
