Do you have experience with skilled nursing or long-term care admissions and enjoy bringing people and teams together?\n\nWe’re looking for a Intake Coordinator to be the critical first step in connecting patients with the right care.\n\nIn this role, you’ll serve as the primary point of contact for new patient referrals, coordinating with facilities, clinical teams, financial partners, and admissions professionals to ensure referrals are processed accurately, efficiently, and with the patient’s needs at the center of every decision.\n\nEssential Duties:\n\n Serve as the primary point of contact for new patient referrals.\nReview referral documentation and screen patients to determine admission eligibility.\nCoordinate the appropriate distribution of viable referrals to facilities that best meet each patient's needs.\nPartner closely with Clinical and Financial teams and facility admissions teams.\nBuild strong, collaborative relationships with facility and admissions teams.\nEnsure referrals are processed promptly, accurately, and confidentially.\nRequirements:\n\n Requirements:\n\nAssociate's degree in business or health related field preferred\n2 years' experience in long term/ skilled care admissions preferred\n\nBenefits include but are not limited to:\n\nMedical, Dental, Vision, and life insurance\n401K\nPet insurance\nCompetitive pay\nCareer advancement opportunities\nEducational opportunities\n\nOur mission is to reconnect people to life through individuals serving individuals.
