BRE is a rapidly growing company that provides ancillary mental and behavioral health services to medical practices. Through our proprietary software and dedicated leased staffing, BRE helps our clients deliver Medicare-supported services to provide more comprehensive care to their senior-aged patients.\nExperience working with senior patients in a family practice medical office, nursing home or skilled nursing facility or similar setting is required. Candidates with a current CNA (Certified Nursing Assistant), CMA (Certified Medical Assistant), or LPN (Licensed Practical Nurse) is required.\nThis is an IN-OFFICE, FULL-TIME position, generally 30-40 hours per week working during regular daytime, clinic hours only (Typically 8am-5pm). NOTE: You must be available for 4-5 full clinic days (Monday-Friday). NO OVERNIGHT OR WEEKEND HOURS AVAILABLE.\nPay commensurate with experience.\nPosition Requirements:\n· CNA (Certified Nursing Assistant), CMA (Certified Medical Assistant), or LPN (Licensed Practical Nurse) REQUIRED\n· Experience working in a family practice medical office, nursing home or skilled nursing facility or similar setting is REQUIRED\n· Work confidentially with the patients following HIPAA compliance standards, in a busy clinic setting\n· The CCM needs to thrive in a highly independent work environment and be confident with using technology\n· Excellent communication and interpersonal skills are a must\n· Well-organized and compliant with completing required tasks\nResponsibilities:\n· Administration of assessments to patients in person\n· Positive, professional and friendly demeanor with senior-aged patients\n· Documenting daily patient notes (per patient) and creating patient activity calendars after each patient encounter\n· Maintaining ongoing, open-lines of REMOTE communication with BRE's Clinical Services Manager and other BRE staff\n· Proficient in Microsoft based software and various web-based software and technology\nJob Type: Full-time\nPay: $24.00 - $26.00 per hour\nBenefits:\n401(k)\nDental insurance\nHealth insurance\nLife insurance\nPaid time off\nVision insurance\nApplication Question(s):\nAre you available for full-time work (roughly 30-40 hrs per week), between M-F, for full clinic days?
