Midland Care ConnectionMidland Care Connection is a not-for-profit, community-based organization dedicated to helping older adults and seriously ill individuals stay independent and supported in their homes. Established in 1978, we offer a full continuum of care, including hospice, palliative care, home health, grief and loss counseling, and our signature PACE (Program of All-Inclusive Care for the Elderly) program. Our mission centers on "meeting individual needs through innovative care solutions," and we serve people across multiple counties in northeast Kansas.PACE Program The PACE (Program of All-Inclusive Care for the Elderly) program is designed to help older adults remain safely in their homes rather than entering nursing facilities. It provides complete medical care, rehabilitation, medications, transportation, social services, and supportive therapies—all coordinated by an interdisciplinary care team. Participants receive personalized care plans tailored to their unique needs, along with access to a PACE center for social activities, meals, and clinical services. By integrating medical and supportive care under one program, PACE helps seniors maintain independence, improve quality of life, and continue living in their communities with dignity.Wellness AssistantA PACE Wellness Assistant supports older adults by promoting strength, mobility, and overall well-being through individualized and group exercise programming. They lead and assist with therapeutic activities designed to improve balance, endurance, and functional independence, while encouraging healthy lifestyle habits and active engagement. The Wellness Assistant monitors participants’ responses to exercise, communicates progress or concerns to the interdisciplinary team, and collaborates closely with therapy staff to support safe and effective wellness interventions. Their role is centered on helping participants maintain independence and enhance quality of life through movement and activity.HoursMonday - Friday | 8:00am - 5:00pmQualificationsRequired QualificationsHigh school diploma or GEDBasic caregiving or healthcare experienceAbility to assist with activities of daily living (bathing, dressing, mobility, transfers)CPR and First Aid certification (or ability to obtain)Physical ability to lift and safely transfer participantsAbility to pass background checks and health screeningsPreferred QualificationsExperience working with older adultsPrevious work in PACE, assisted living, home health, or similar settingsKnowledge of elder care and dementia supportStrong communication and teamwork skillsWhat’s in it for you?
