Prevention services utilize comprehensive family assessment tools to identify behaviors related to risk and to develop Core Outcomes that address the identified child welfare concerns within a family.\nThe Case Planner provides in-home counseling services to families in need of support in developing skill and competencies that will enable them to keep their children in the home and out of placement. The Case Planner will maintain a caseload of 12-14 families and provide change focused interventions with families to improve parental functioning, address life stressors, and improve overall familial functioning. This is happen over a course of 6 to 12 month length of service and a minimum of 2 face to face contacts with each family per month. The Case Planner will have to have at least 1 late night per week to meet the needs of working families.\nJob Responsibilities:\nProvide weekly change focused interventions with families, primarily in the home, however sessions can take place at the location that best meets the family’s needs during that week, such as at school, the local park, day care center or at Astor offices.\nProvide counseling to improve parental functioning, address life stressors and improve overall familial functioning.\nAssess the safety of all children in the home and actively intervene to ensure child safety.\nAssess all children along the following domains: physical development, cognitive development, emotional development and social/behavioral functioning.\nAssess the well-being of all family members, including the impact of domestic violence, substance abuse and/or mental health problems on each member’s functioning.\nMaintain contact with all providers of collateral services, including schools, day care programs, mental health service providers, substance abuse treatment service providers and probation officers as needed.\nProvide crisis intervention as needed.\nDevelop service plans in collaboration with the family.\nWhat we provide:\n4 weeks paid vacation annually.\n13 paid holidays.\n4 personal days.\n1 sick day per month, accruable to 150 days.\nFully paid individual LTD and life insurance.\nEligible for participation in agency medical and dental plan both individual and family, with small employee contribution.\nNew longevity incentives.\nOpportunities to make a difference in the lives of children and families in need.\nCollaboration with a team of dedicated professionals in a supportive and dynamic environment.\nRoom for growth and advancement within the organization.\nSalary: $42,642 - $46,906/year\nJoin Astor Services – Apply Now!\nDo you meet the qualifications above?
