In this role, you will help clients receive quality care in community-based settings rather than institutional environments.With over 21 years of service, TCS is a nonprofit organization committed to person-centered care, regulatory excellence, and community impact.Compensation & BenefitsBase Salary: $60,000Quarterly Bonus: Up to $1,250 based on performancePublic Service Loan Forgiveness (PSLF) eligible employerHybrid work environment (remote + local travel)Mileage reimbursementBenefits Package Includes:Medical (Kaiser), Dental and Vision (Mutual of Omaha)401(k) with matchingPaid Time Off (PTO)Flexible Spending Account (FSA)Company-paid Life, AD&D, and Short-Term DisabilityFlexible scheduling optionsEmployee discountsEmployee referral program (Bonus of $1,000)About the RoleAs a Case Manager – Medical Day Care Services, you will provide person-centered case management to individuals (age 16+) who meet nursing facility level of care but choose to live in the community.Key ResponsibilitiesConduct assessments using Person-Centered Planning (PCP)Develop and manage individualized service plansCoordinate Adult Medical Day Care and community-based servicesDocument all service coordination activities in LTSS Maryland dailyConduct in-person client visitsMonitor service effectiveness and client outcomesEnsure compliance with program and regulatory requirementsMaintain accurate and timely documentationQualificationsRequired:Bachelor's degree in Human Services or related field (or approved equivalent)Some experience in case management or service coordinationStrong communication, documentation, and organizational skillsValid driver's license, reliable transportation, and safe driving recordAbility to travel locally throughout MarylandMust be at least 21 years oldAbility to pass background checksAuthorized to work in the United StatesPreferred:Experience with Adult Medical Day Care or Medicaid Waiver programsFamiliarity with LTSS Maryland systemExperience working with individuals with complex medical or behavioral needsScheduleFull-time, Monday through FridayFlexible based on client needsAbout Total Care Services, Inc.Total Care Services, Inc. is a nonprofit healthcare organization providing community-based services across Maryland.
