Working under the supervision of a Registered Nurse, you will:Deliver high-quality skilled nursing care according to physician orders and individualized care plansMonitor patient conditions and identify changes requiring interventionProvide wound care, medication management, disease education, and other skilled nursing servicesEducate patients and caregivers on self-care techniques, treatment plans, and safety measuresCollaborate closely with RN Case Managers, physicians, therapists, and other members of the care teamDocument patient visits accurately and timely in the electronic medical recordAssist in implementing care plans designed to improve outcomes and reduce avoidable hospitalizationsCommunicate significant findings and patient concerns to the interdisciplinary teamPromote patient independence, safety, and quality of life in the home settingSchedule & CoverageFull-time positionFlexible scheduling with support from local clinical leadershipWhat You'll NeedGraduate of an accredited Practical Nursing programCurrent, unrestricted Licensed Practical Nurse (LPN) licenseMinimum of one (1) year of nursing experience in a clinical settingValid driver's license, reliable transportation, and current auto insuranceAbility to travel throughout the assigned service areaStrong communication, assessment, and organizational skillsPhysical Requirements: Sit, stand, bend, lift and move intermittently and be able to lift 50-100 lbs.Preferred QualificationsPrevious Home Health experienceExperience working with adult and geriatric patient populationsExperience with electronic medical record systemsJoin Our MissionAt Elara Caring, every team member plays a vital role in helping patients receive the care they need in the place they call home.
