Registered Nurse Care Manager (Pediatric)
Job TitleFacilitates the patient's hospitalization from admission through discharge.Job DescriptionCoordinates with physicians, other nurses and social workers and other disciplines within the care team, including outside agencies, to expedite the appropriateness, effectiveness and timeliness of care.Job ResponsibilityProvides health education; stratify and Manage riskCoordinate care with internal and external providersActively participate with a multidisciplinary teamConduct assessments and Develop treatment plansApplies clinical expertise and medical appropriateness criteria to resource utilization and discharge planning, and manages the resources necessary for cost effective, quality patient care. Coordinate care with internal and external providers with a wide range of professionals including psychiatrists, nurse practitioners, community support specialists, nurses, on-site pharmacist, social workers, licensed professional counselorsPartner with the Access team to triage new and existing clients, respond to emergent and urgent medical and psychiatric needs, and provide direct careJob QualificationGraduate from an accredited School of Nursing. Bachelor's Degree in Nursing, preferred. Must be enrolled in an accredited BSN program within two (2) years and obtain a BSN Degree within five (5) years of job entry date.Current License to practice as a Registered Professional Nurse in New York State required, plus specialized certifications as needed.Pediatric experience, strongly preferred.Additional Salary Detail The salary range and/or hourly rate listed is a good faith determination of potential base compensation that may be offered to a successful applicant for this position at the time of this job advertisement and may be modified in the future.
