The LPN will support clinical workflow in an extremely busy medical clinic, be responsible for providing quality patient care, assisting providers, and ensuring efficient day-to-day clinic operations.\n\nMinimum Requirements\n\nLicense to practice as a Licensed Practical Nurse in the State of Louisiana\nBe over 18 years of age.\nPossess a valid driver’s license\nCurrent CPR certification\nPrior experience preferred\n\nCore Competencies\n\nParticipates on a multidisciplinary team by providing primary and/or behavioral health care to clients according to the scope of licensure.\nAdministers and monitors medications according to prescriber’s orders.\nIdentifies and reports any concerns related to medication adherence, misuse, and/or adverse reactions.\nPromotes recovery, independence, and personal choice for participants.\nAssists clients in identifying strengths, needs, and realistic goals to develop a treatment plan.\nDemonstrates advanced knowledge of primary and behavioral health symptoms, diagnoses, interventions, and impact.\nMaintains professional boundaries and operates within agency / ethical guidelines.\n\nJob Duties / Skills Required\n\nProvide nursing care within scope of practice that is patient centered and individualized and utilizes the nursing process of assessing, planning, implementing and evaluating.\nParticipate in setting treatment plan goals/objectives. Conducts healthcare screenings and assessments.
