Registered Nurse - PRN Home Infusion - Scottsdale

Job description Sisu Healthcare Solutions is a privately held infusion therapy company based in Tempe, AZ. We are dedicated to providing excellent clinical service to our patients, physicians, and managed care customers. Our comprehensive infusion therapy services include the provision of nutritional, antibiotics, and specialty drugs administered intravenously in the home setting or in our outpatient ambulatory infusion suites. Our clinical team of nurses and pharmacists work closely with physicians to develop personalized plan of care and ensure optimal patient outcomes. Our specially trained nurses assess the home setting, administer the medication, and teach caregivers in the home. From pharmacokinetic monitoring to reviewing medication profiles our pharmacists are working closely with patients as well as their physicians. Home infusion therapy has proven to be a cost-effective alternative to hospitalization or hospital-based outpatient care. Job Description Provide a broad range of infusion therapies to patients in the comfort of their own home. Initiates, develops and implements nursing plan of treatments. Assesses patient needs and physical status at each visit through health data systems and patient review. Re-evaluates patient needs through physical reassessment, response to therapy, and supplemental physician orders. Provides effective and safe teaching for patients and families to achieve independence with prescribed therapy and care needs through active participation per plan of treatment goals. Have excellent written and verbal customer service skills to interact with the patient, physician, pharmacist and other partners. Communicate with the clinical team, medical providers, patients, and families/caregivers to facilitate continuity of care. Prepares clinical documentation in real-time during visits and submits to the nursing department to comply with established timelines. Coordinates discharge planning and prepares discharge summaries with patient instructions and thoroughly reports patient care needs, progress and goals when transferring care. Demonstrates compliance with agency operations, SISU Healthcare Solutions policies and procedures, professional standards, local, state, federal regulations/guidelines, and accreditation standards. Maximizes work efficiency using computers and other automation technologies to validate plan of treatment orders, visit records and follows assigned visit schedule Observes legal and ethical guidelines for safeguarding the confidentiality of patient and proprietary Sisu Healthcare Solutions information including adherence to HIPAA regulations. Actively participates in facility process for Performance Improvement Program and assists Supervisor or Nurse Manager with chart audit functions as applicable. In this role, you will work a flexible weekly schedule concentrating on patients in Scottsdale/Central Phoenix, AZ and surrounding areas which may require travel of a 1-hour radius outside the normal territory (travel time is paid and mileage program). You will be required to participate in the on-call rotation as indicated by staffing and business needs. Required Qualifications Registered Nurse with current unrestricted license in state of Arizona. Minimum two-year nursing experience; IV-related experience a plus Have and maintain current CPR certification Possess a valid and current driver’s license and auto insurance Have experience with Microsoft Office, Excel, Outlook and Word to document and track patient care activity. Participate in on call rotation as indicated by staffing and business needs. Pay: $42.00 - $43.00 per hour Benefits: Flexible schedule Work Location: On the road
