Group Home Management\n\n\\* Oversee the day-to-day operations of the Care Base LLC group home.\n\\* Ensure the home remains safe, clean, organized, and comfortable for individuals served.\n\\* Ensure individuals receive services according to their approved service plans and identified needs.\n\\* Coordinate daily activities, appointments, transportation, meals, personal care, and community activities.\n\\* Promote independence, dignity, choice, community participation, and quality of life.\n\n### 2. HCBS WAIVER SERVICES & COMPLIANCE\n\n\\* Demonstrate working knowledge and experience with \\*\\*Home and Community-Based Services (HCBS) waiver programs\\*\\*.\n\\* Ensure services are delivered in accordance with applicable HCBS requirements.\n\\* Monitor compliance with individual service plans and authorized services.\n\\* Ensure required documentation is accurate, complete, timely, and maintained appropriately.\n\\* Monitor incident reporting, medication documentation, progress notes, service records, and other required documentation.\n\\* Assist Care Base LLC in preparing for audits, inspections, licensing reviews, and quality-assurance activities.\n\\* Identify compliance concerns and implement corrective actions when necessary.\n\n### 3. STAFF SUPERVISION\n\n\\* Supervise and provide leadership to direct-support professionals and other residential staff.\n\\* Develop staff schedules and ensure appropriate staffing coverage.\n\\* Train, coach, and mentor employees to provide quality services.\n\\* Monitor staff performance, attendance, professionalism, and compliance with company policies.\n\\* Conduct staff meetings and provide ongoing guidance and training.\n\\* Address employee performance issues appropriately and communicate significant concerns to management.\n\\* Ensure staff maintain required training, certifications, and qualifications.\n\n### 4. INDIVIDUAL SUPPORT & CARE COORDINATION\n\n\\* Ensure individuals are treated with dignity, respect, compassion, and fairness.\n\\* Monitor individuals' physical, emotional, behavioral, and social well-being.\n\\* Coordinate medical, dental, therapy, psychiatric, and other appointments as needed.\n\\* Communicate appropriately with guardians, families, case managers, healthcare providers, and other members of the individual's support team.\n\\* Participate in service planning meetings and advocate for the needs and rights of individuals.\n\\* Report significant changes in an individual's condition or needs promptly.\n\n### 5. HEALTH, SAFETY & MEDICATION OVERSIGHT\n\n\\* Maintain a safe residential environment for individuals and staff.\n\\* Ensure staff follow approved medication administration procedures.\n\\* Monitor medication records and promptly address medication errors or concerns.\n\\* Ensure emergency procedures and safety plans are maintained and followed.\n\\* Ensure incidents, accidents, injuries, suspected abuse, neglect, exploitation, or other serious concerns are reported according to applicable requirements.\n\\* Respond appropriately to emergencies and urgent situations.\n\n### 6. DOCUMENTATION & RECORDS\n\n\\* Review daily staff notes and service documentation for accuracy and completeness.\n\\* Ensure documentation supports services actually provided to individuals.\n\\* Review medication administration records and other required records.\n\\* Ensure incident reports are completed and submitted within required timelines.\n\\* Maintain confidentiality of individual and employee information.\n\\* Correct documentation deficiencies and provide staff with appropriate direction.\n\n### 7. QUALITY ASSURANCE\n\n\\* Regularly monitor the quality of services provided at the group home.\n\\* Conduct routine reviews of staff performance, documentation, health and safety, and residential conditions.\n\\* Identify areas for improvement and develop corrective action plans.\n\\* Promote continuous improvement in services and individual outcomes.\n\\* Ensure Care Base LLC maintains high standards of person-centered care.\n\n## REQUIRED EXPERIENCE & QUALIFICATIONS\n\nThe ideal candidate should have:\n\n\\* Experience managing or supervising a \\*\\*group home, residential program, or HCBS service program\\*\\*.\n\\* Demonstrated experience working with \\*\\*HCBS waiver services\\*\\*.\n\\* Experience supporting individuals with \\*\\*intellectual and developmental disabilities (I/DD)\\*\\*.\n\\* Experience supervising direct-support professionals or other healthcare/human-services staff.\n\\* Knowledge of person-centered planning and individual rights.\n\\* Knowledge of service documentation and incident reporting.\n\\* Understanding of residential health and safety requirements.\n\\* Strong organizational and leadership abilities.\n\\* Excellent written and verbal communication skills.\n\\* Ability to work effectively with individuals, families, guardians, case managers, healthcare professionals, and employees.\n\\* Ability to maintain confidentiality and professional boundaries.\n\\* Ability to respond calmly and appropriately to emergencies and challenging situations.\n\n\\*\\*Education:\\*\\* A bachelor's degree in Human Services, Social Work, Psychology, Healthcare Administration, or a related field is preferred.
