Case Manager - ID

GENERAL JOB DESCRIPTION The Case Manager for ID assists in the full implementation of social program needs with a special focus on resolving barriers to engagement in services, ensuring linkage to medical and behavioral health care, with overall goal of increasing client's ability to self-manage while facilitating integration of care for those individuals and families working within a multiple services systems. The ID Case Manager will assess all new and existing clients for grant program eligibility, answer and respond to inquiry calls, and provide program information and referrals for new and existing clients. DUTIES & RESPONSIBILITESProvides advocacy and linkage to care that will help clients increase personal and family stability, and achieve personalized sustainable care plan goalsAssists in development, implementation, and revision of individual treatment plans. Assures that services provided are specified in the Treatment Plan and monitors progress toward treatment goals when necessaryServes as a liaison between the patient and the care team as appropriateFacilitates social, peer, and community supportFollows up on no show appointments to ensure clients are not lost to careHelps and empowers clients to develop and utilize independent living skills and strategiesReceives documentation from providers and follows up with patients to assist in linking patients to resources.Reviews grant program eligibility with clients and assists with appropriate applications and deliverablesProvides recommendations for appropriate grant assistance programsSchedules and attends meetings with providers and patients as neededInforms all parties involved in processes that used to treat and/or link patients to servicesMaintains patient confidentiality in treatment plans and reporting as appropriate. Understands and abides by HIPAA guidelinesCommunicates with patients regularly via phone or in person to monitor their well-being, address their concerns, and connect them with various resourcesProvides for face-to-face meetings or phone contact and home visits as determined by client needMaintains patient's electronic health records and hard copy forms in accordance with applicable standards and regulations, grant requirements, etc.Maintains a high level of ethical conduct regarding confidentiality, dual-relationships, and professional statureEnters applicable grant services in state of Louisiana's CareWare reporting system.Must become certified to perform Rapid HIV Testing & Counseling as needed for community outreach eventsAttends meetings as appropriate and meets regularly with supervisor to exchange pertinent information and receive guidanceResponsibilities not limited to those listed. Occasional work at outdoor events with varying temperatures as necessary. Bachelor's degree (preferably in a healthcare or social sciences related field, ie: Liberal Arts, Psychology)2 years of case management experienceExperience in collaboration with interdisciplinary healthcare teamsGood interpersonal and communication skillsReliable Transportation (work would entail traveling to clinic locations and social service agencies in the community)Flexible scheduling including weekends requiredPREFERRED QUALIFICATIONSMaster's degree (preferably in a healthcare or social services related field)Experience in HIV/STI programsBilingual (English/Spanish) desirableKnowledge of Ryan White grant programKnowledge of CAREWare software application
