Medical Doctor

CHM Mobile Care, based in Shelbyville, KY, is seeking a qualified Physician to join our growing practice. In this role, you will collaborate with Advanced Practice Nurse Providers in providing direct patient care to individuals transitioning from acute to post-acute and in long-term care.This role will perform H&P on new admissions, round on existing patients, evaluate and assess patient conditions, order labs and prescriptions, and provide recertifications and discharge orders.To qualify for this role, you will have:MD or DO from an accredited medical university.Active and unencumbered license to practice Medicine in Kentucky.Active and unencumbered DEA license to prescribe in Kentucky.Active BLS or ACLS certification.Board Certification in Internal, Geriatrics, or Family Medicine.Excellent communication and collaboration skills.Ability to see patients at a minimum of 2 days per week.Ability to travel within an hour radius of home.
