This position focuses on monitoring patient data and alerts, communicating with patients by phone, and identifying situations that require clinical follow-up or escalation.\n\nThis is a great opportunity for an RN who is *tech-savvy, organized, clinically confident, and comfortable working independently in a remote environment.*\n\nWhat You'll Do\n\n* Monitor and respond to *RPM alerts* and patient monitoring data.\n* Review patient readings and identify abnormal or concerning trends.\n* Call patients regarding alerts, abnormal readings, missed readings\n* Escalate clinical concerns to the appropriate provider or care team\n* Accurately document patient calls, assessments, interventions, and escalations.\n* Complete patient outreach and follow-up in a timely manner.\n* Manage multiple patients, alerts, and tasks\n* Maintain HIPAA compliance and patient confidentiality.\n\nRequirements\n\n* *Active RN license with a Multistate/Compact license.*\n* Strong clinical assessment and critical-thinking skills.\n* Excellent communication and phone skills.\n* Strong computer and technology skills.\n* Ability to work independently in a remote environment.\n* Excellent organization and attention to detail.\n* Ability to efficiently manage multiple tasks and computer platforms.\n* Reliable internet connection and an appropriate remote work environment.\n\n*Strongly Preferred*\n\nRNs who have a *Multistate/Compact license PLUS an active license in any of the following states are strongly preferred:*\n\n* *Illinois (IL)*\n* *California (CA)*\n* *Hawaii (HI)*\n* *Oregon (OR)*\n\nAdditional state licenses may be *required after hire* based on patient populations and business needs. Candidates must be willing to obtain additional state RN licenses as needed.\n\nRemote/Technology Experience\n\nPrevious *remote nursing, telehealth, RPM, CCM, triage, or patient outreach experience* is a plus.\n\nOur clinical team works across a variety of computer systems, electronic medical records, communication platforms, and clinical applications.
