Job Description Job Opportunity: Weekend Physical TherapistOur organization is on the lookout for a dedicated Weekend Physical Therapist to join our Home Health team. This position primarily involves working during weekends to provide exceptional rehabilitative care directly in patients' homes.Key ResponsibilitiesConduct thorough physical therapy evaluations and treatments for adult and elderly patients recovering from surgery, illness, or injury, as well as those managing chronic conditions.Travel to patients' homes to assess their functional mobility and ensure home safety.Develop and implement personalized care plans tailored to each patient's needs.Utilize evidence-based therapeutic techniques to enhance strength, balance, endurance, pain management, and overall independence.Work collaboratively with nurses, occupational therapists, speech therapists, and physicians to coordinate care and modify treatment plans based on patient progress.Educate patients and their caregivers on home exercise routines, safe mobility practices, fall prevention strategies, and the effective use of assistive devices.Maintain accurate and timely documentation using electronic medical records to ensure compliance with regulatory standards and quality benchmarks.Requirements and QualificationsLicensed Physical Therapist with experience in home health care.Ability to work independently in the field and manage a caseload focused on weekend hours.Strong clinical judgment and excellent communication skills.Reliable transportation for travel to patient locations.Comfortable assessing and treating patients with complex medical needs.Working ConditionsThis role offers a concentrated work schedule primarily on weekends, allowing for full-time pay and benefits while providing flexibility for clinicians.
