No weekends.Location: OhioWe are seeking a mission-driven Family Medicine Physician to join a Federally Qualified Health Center (FQHC) providing comprehensive, patient-centered primary care to individuals and families across the lifespan. This role focuses on delivering high-quality, culturally competent care to medically underserved and vulnerable populations in a collaborative, community-based setting.Key ResponsibilitiesProvide comprehensive primary care services to patients of all ages, from pediatric to geriatric populationsDiagnose and treat acute and chronic medical conditionsManage chronic diseases such as diabetes, hypertension, asthma, and cardiovascular diseaseDeliver preventive care services, including screenings, immunizations, and health educationPerform in-office procedures such as suturing, skin biopsies, and joint injectionsProvide women's health services, including Pap smears and contraception counselingOrder and interpret laboratory and diagnostic testingPrescribe medications and monitor treatment plansPopulation Health & QualityParticipate in quality improvement initiatives and performance metric reporting (e.g., UDS measures)Support population health strategies for high-risk and medically complex patientsCollaborate with care coordinators, behavioral health providers, and case managersPromote preventive care and health education within the communityTeam-Based CarePractice within a Patient-Centered Medical Home (PCMH) modelCoordinate care with specialists, hospitals, and community resourcesSupervise or collaborate with nurse practitioners, physician assistants, and medical residents as applicableCompliance & DocumentationMaintain accurate and timely documentation in the electronic health record (EHR)Adhere to FQHC regulations and HRSA compliance requirementsSupport sliding fee scale policies and access-to-care initiativesQualificationsDoctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO)Completion of an accredited Family Medicine residency programBoard Certified or Board Eligible in Family MedicineActive, unrestricted state medical licenseDEA registrationCurrent BLS and ACLS certification BenefitsHealth Insurance: Medical - Employer Pays 70%Dental & Vision coverageShort Term Disability /Long Term DisabilityEmployer Paid Life Insurance401K w/ 4% Match4 weeks of PTO7 paid HolidaysCME, licensing, DEA, travel reimbursementFTCA malpractice coverage
