This role offers the opportunity to work independently, manage complex medical conditions, and build strong, trusted clinical relationships — all while being supported by a collaborative interdisciplinary team.\r\nYou will serve as a consistent clinical presence within your assigned facilities, providing proactive, patient-centered care and responding to urgent medical needs to ensure continuity, quality, and dignity in treatment.\r\nRoles and ResponsibilitiesProvide primary, urgent, and preventive medical care to residents in skilled nursing and, as applicable, assisted living settings.\r\nIndependently evaluate, diagnose, and treat acute and chronic medical conditions.\r\nConduct initial admissions and follow-up assessments, including physical exams, medication review, and clinically appropriate screenings.\r\nMonitor changes in resident condition and adjust treatment plans accordingly.\r\nOrder and interpret diagnostic tests; prescribe medications and therapies within scope of practice.\r\nCollaborate with facility Medical Directors, attending physicians, nursing leadership, and interdisciplinary teams.\r\nCommunicate clearly and compassionately with residents and families regarding diagnoses, treatment plans, and changes in condition.\r\nMaintain accurate and timely documentation in the electronic medical record in accordance with regulatory and payer standards.\r\nParticipate in quality and care coordination efforts to support excellent patient outcomes.\r\nAdhere to HIPAA, state, and federal regulations while upholding ethical standards of clinical practice.\r\nMinimum QualificationsActive, unrestricted Nurse Practitioner license in the applicable state(s) of practice.\r\nCurrent national Nurse Practitioner certification (ANCC, AANP, or equivalent).\r\nClinical training and experience in adult, family, and/or geriatric medicine.\r\nEligibility to work in the United States.\r\nPreferred ExperienceExperience in internal medicine, geriatrics, cardiology, or post-acute care.\r\nActive DEA registration or ability to obtain.\r\nStrong clinical judgment and ability to work independently.\r\nExcellent communication and interpersonal skills.\r\nOrganized, detail-oriented, and able to manage multiple priorities effectively.\r\nPrior experience in SNF, LTC, ICU, or Emergency Department settings is a plus.\r\nAbility to collaborate effectively within interdisciplinary care teams to optimize resident outcomes.\r\nOur Core Values in PracticeServeStrong relationships drive strong care and overcome adversity together.\r\nTeam PlayerWe support each other, bring all the facilities resources together, communicate openly, and solve problems together\r\nTenacityOur passion for service creates an intensity to serve with you!
