As a Stonerise Home Health Physical Therapist (PT), you will be responsible for providing physical therapy services and are responsible for the supervision of corresponding team members. Some of your primary responsibilities include:Providing physical therapy services to patients according to the patient plan of care which may include assessing and evaluating therapeutic/rehabilitative/functional status, assessing for muscle strength, mobility, gait, ROM, etc.Initiating physical therapy programs and instructing other team members and/or families/caregivers in certain phases of the therapy with which they may work with the patientTraining patients on the use of prosthetic devicesIdentifying patient and family/caregiver needs for other home health services and referring as necessaryPreparing and submitting clinical and progress summaries based on patient goalsParticipating in discharge planning of patientsCompleting a 30-day functional reassessment and revising plan and goals accordinglyProviding physical therapy consultation to home families/caregivers as neededSupervising and directing physical therapy assistants (PTA)Benefits You Will EnjoyPaid Time OffInsurance benefit package (medical, dental, vision, life, and other voluntary group programs)401 (K) retirement savings planTuition reimbursementAdvancement opportunitiesTraining, development & continuing education opportunitiesStonerise Chaplain supportEmployee Assistance Program (EAP)Schedule Non-ExemptEducation, Licensure & Certification RequirementsPossesses a baccalaureate level degree or higher in physical therapy by an accredited organizationLicensed to practice as a Physical Therapist within the stateAt least two (2) years of experience as a PT. Community/home health experience preferredValid Driver's License and an automobile that is insured and in good working order
