Our therapy team works with residents through real recovery milestones: post-surgical rehab, stroke recovery, fall prevention, and return-to-home planning.This isn't a high-pressure hospital caseload. You'll have time to do the work right.The RoleYou'll carry a structured caseload of skilled nursing residents, developing and executing individualized PT plans that directly impact mobility, functional independence, and discharge outcomes.Your day-to-day includes:Initial evaluations and goal-setting for new admissionsTherapeutic exercise, gait training, balance work, and functional mobilityTransfer training and assistive device assessmentCoordinating with nursing, OT, SLP, and care teams on shared resident goalsFamily education on home safety, carry-over exercises, and discharge prepDocumentation in compliance with Medicare and facility standardsNo surprises.
