You will work within a collaborative, supportive team structure, learn new ways to provide optimal supports to your clients and help people change their lives for the better!Position SummaryEstablishes relationships with housing resources in the community to assist members in obtaining housing. Supports members in learning and applying skills to maintain housing. Helps members access housing resources across the continuum of housing options so they can achieve an optimal level of health and functioning in the community. Provides Housing Transition Navigation Services, Housing Deposits Supports, and Housing Tenancy and Sustaining Services.This position is considered essential, including during times of public or community emergencies, including but not limited to a pandemic, earthquake, wildfire, or other event. Specific job duties, services, and adaptations will be determined as business needs dictate and as directed by a supervisor.Essential FunctionsForms and fosters relationships with housing agencies and permanent housing providers, including supportive housing providers.Works directly with the CRF Housing Department to identify and partner with community resources to ensure timely and seamless access to housing support services.Partners with housing agencies and providers to offer the ECM/CS member permanent, independent housing options, including supportive housing.Connects and assists the ECM/CS member to access available permanent housing, which includes completing the VI-SPDAT assessment and entering it into the County of San Diego Coordinated Entry System (CES).Provides mobile services in which the Housing Specialist coordinates with the member in the most easily accessible setting.Assists member(s) in maintaining housing once established by monitoring residence compliance with program rules and regulations; addresses facility problems with the landlord or owner; assists member(s) with financial planning resources to ensure rent and utilities are paid in a timely manner; provides mediation and conflict resolution between member(s) and other tenants if needed.Works individually to help members identify housing goals and barriers and creates an action plan to achieve such goals.Continually identifies housing resources across the housing continuum to meet member needs.Participates in community housing development activities and planning as related to the interests of members and the CRF mission.This position requires being eligible and able to drive company vehicles and or transport members in a personal vehicle to provide field-based services. Employee may be assigned to work in one region of San Diego, but as needed will have to travel to all regions of San Diego.QualificationsEducation/Certification: Bachelor’s Degree in Public Health, Social Work, Psychology, Human Services, or related field required. MA or enrollment in MA program in related field strongly preferred.Background/Experience: 1 year experience working in social services or related setting required. Previous experience in Housing placement in social services setting strongly preferred. Experience working with individuals with serious mental illness or chronic conditions preferred. A valid California driver’s license in good standing and eligibility to be on the CRF insurance carrier Approved Driver’s List is required to fulfill the essential functions of this position. In addition, maintaining proof of valid and adequate insurance is required to be able to drive personal vehicles on company business.  Proficiency with MS Office applications; ability to navigate various EHRs and use electronic health technology.CRF Behavioral Healthcare | Employment
