Care Coordination Specialist

Coordinates the daily activities of the patient care unit and keeps the Nurse Manager or Director up-to-date on all activities. Assesses, plans, implements and evaluates the nursing care of patients. Assists in establishing and monitoring patient care standards, and in the absence of the Nurse Manager assumes that role.Key ResponsibilitiesCoordinate daily clinical activities across departments to ensure smooth patient care deliveryAct as a liaison between physicians, nursing staff, and administrative teamsMonitor patient flow, admissions, discharges, and transfersEnsure adherence to clinical protocols, safety standards, and regulatory guidelinesSupport quality improvement initiatives and patient care programsMaintain accurate clinical documentation and reportingAssist in staff scheduling, training coordination, and performance monitoringHandle patient and family concerns, ensuring high satisfaction levelsCollaborate with departments such as emergency, ICU, and outpatient servicesSupport audits, inspections, and accreditation processes
