Our Lady of Consolation is located on the beautiful Great South Bay and is comprised of a 345-bed nursing and rehabilitative care center. The Hospital Assessment Liaison serves as the primary connection between hospitals, patients, families, and the nursing home admissions team. This role is responsible for coordinating referrals, reviewing clinical information, completing patient assessments, and facilitating smooth admissions and transitions into the skilled nursing facility. The liaison helps ensure appropriate placement while maintaining strong relationships with hospital discharge planners, case managers, and healthcare providers

Key Responsibilities
Serve as the main point of contact for hospital referrals and potential admissions.
Review patient medical records, insurance coverage, and clinical needs to determine admission eligibility.
Conduct bedside or virtual assessments of prospective residents in hospitals or other healthcare settings.
Collaborate with hospital case managers, social workers, physicians, and nursing staff regarding patient discharge plans.
Coordinate admissions with nursing, rehabilitation, dietary, social services, and administration teams within the facility.
Educate patients and families on services, insurance coverage, rehabilitation programs, and long-term care options.
Ensure all required admission documentation is complete and compliant with state and federal regulations.
Maintain ongoing communication with referral sources to build and strengthen professional relationships.
Track referral outcomes, admissions data, and occupancy goals.
Participate in marketing and outreach efforts to hospitals and healthcare providers.
Assist with resolving admission concerns or barriers to placement.
Stay informed on Medicare, Medicaid, managed care, and insurance authorization requirements.
Qualifications
Active New York State Registered Nurse or MSW License.
Experience in skilled nursing, hospital discharge planning, admissions, case management, or rehabilitation services preferred.
Knowledge of long-term care regulations, insurance authorization processes, and post-acute care services.
Strong interpersonal, communication, and organizational skills.
Ability to review clinical documentation and make appropriate placement recommendations.
Proficiency with electronic medical records (EMR) systems and strong computer skills.
Valid driver's license and ability to travel to hospitals or referral sites as needed.
Salary Range
USD $52.25 - USD $73.15 /Hr.
The pay for this position ranges from $52.25-$73.15 per hour.
This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate's qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses/incentive, differential pay or other forms of compensation that may be applicable to this job and it does not include the value of benefits.
