Scans and validates all loose reports that are sent to the department. Responsible for analyzing medical records for deficiencies in chart documentation. Responsible for release of information for continuity of care requests. Assures that guidelines of HIPAA are followed to reduce the hospital’s potential liability in releasing records or protected health information. Responsible for processing requests for government audit and specialized chart reviews (UVMC only). Responsible for monitoring incomplete charts and following up with physicians. Responsible for following guidelines of Joint Commission and Conditions of Participation and all federal and state regulations. Responsible to perform other duties as assigned and needed.\nQualifications Education: Minimum Level of Education Required – High School or GED.\nLicensure/Certification/Registration: Medical Terminology.\nExperience: Minimum Level of Experience Required – 1 to 3 years of job‑related experience.
