Position Overview We are seeking experienced and compassionate Registered Nurses (RNs) to perform in-home visits across all NYC boroughs. As a Fee-for-Service Registered Nurse, you will play a vital role in patient care, assessments, and care coordination—helping clients live safely and comfortably in their homes. Compensation (Per Diem Rates) Initial Assessment Visit:$130 Reassessment Visit:$100 Resumption of Care Visit:$100 Supervisory Visit:$80 Key Responsibilities Conduct initial and follow-up in-home nursing assessments Develop and update individualized care plans Educate patients and families on disease management and self-care Supervise and instruct caregivers and home health aides as needed Complete timely and accurate documentation in compliance with regulations Communicate effectively with interdisciplinary team members Qualifications Active New York StateRegistered Nurse (RN)license 1–2 years of experience in CHHA or long-term care preferred Strong clinical assessment and documentation skills Professional liability insurance is required Reliable transportation and willingness to travel between boroughs Apply Today Join our dedicated team of Registered Nurses making a real difference in the lives of New Yorkers.
