Ensures that work environments are safe, secure and where individuals are free from harm.\r\nSupervises and performs case management responsibilities, including preparation, implementation an evaluation of Individual Support Plans for employees funded by the Department of Developmental Services, Massachusetts Rehabilitation Commission, School Departments, etc. Works closely with the DDS Case Management Team Service Coordinators.\r\nWill serve as the program Safety Officer as assigned by Director and be responsible for implementing safety plans and programs.\r\nEncourages regular communication and cooperation between staff of all LES programs to ensure coordination of rehabilitation and production efforts. Maintains effective communication with residential providers and families regarding activities and supported employment placements.\r\nCoordinates the scheduling of daily and weekly staff and employee work assignments, for all individual, enclave supported employment placements and Life Enhancement activities. Coordinate with Transportation Department the weekly transportation schedule for all employment sites.\r\nCoordinate evaluation and intake of referrals with leadership team.\r\nProvide coverage support as necessary for various sites, placements, activities and outings in the community.\r\nProvide training, support and follow-up to individual and group placements.\r\nMaintain accurate records for individuals served for the purpose of program evaluation, billing etc.\r\nCoordinate accurate payroll records for all individuals and employees, job coaches and relief staff.\r\nAssist leadership team in long-term and short-term planning development, organization and evaluation of services and in the daily operation of program.\r\nImplements Quality Enhancement standards within the program.\r\nCoordinate the scheduling of all relief staff for the Program.\r\nOther duties and/or assignments to work/program locations based on the needs of the company.\r\nRequirements\r\nExperience working with Individuals with ID/DD\r\nBachelor's degree from four-year college or university and one to two years related experience\r\nMust be able to pass Fingerprint background check, CORI, DPPC and Driving background checks\r\nValid Driver's License required\r\nExcellent verbal and written communication skills\r\nExcellent Benefits Package\r\n403(B) matching up to 3%\r\n$2,000.00 Staff Retention Bonus after 1 year**\r\nHealth Insurance with 2 plan choices and a 75% employer contribution\r\n4.4 weeks of Paid Time Off in the first year of employment\r\nLong-Term Disability insurance & Life Insurance free for employees\r\nTuition reimbursement\r\nFlexible spending account\r\nLifeworks, Inc. believes diversity strengthens the value of our workplace and communities.
