Travel Physical Therapist - Home Health
American Traveler is seeking a licensed Physical Therapist with a minimum of 2 years of experience for a 13-week home health/hospice assignment with a Monday-Friday schedule and occasional weekend coverage.Job Details• Home health and hospice setting serving patients in the southwest region,• Schedule is Monday through Friday, 8:00 AM to 4:00 PM, with 2-3 weekend days per assignment,• Caseload includes home-based patients requiring comprehensive PT assessments, plan of care development, and ongoing skilled treatment,• Responsibilities span OASIS assessments, wound care, staple/suture removal, and PT/INR and blood pressure monitoring,• Uses McKesson for clinical documentation, including visit lock-downs, follow-up notes, and OASIS time points,• Involves coordination with a multidisciplinary care team including physicians, nurses, rehab staff, and home care aides,Job Requirements• Current state PT license required if applicable,• Minimum 2 years of physical therapy experience required,• Home health experience strongly preferred given OASIS assessment responsibilities,• Familiarity with McKesson EMR preferred,• Experience completing OASIS documentation in accordance with state and federal guidelines,• Knowledge of APTA guidelines and Medicare/insurance billing and coverage requirements,• Ability to independently manage and adjust a daily patient visit schedule,• DOB and SSN may be required for consideration,Additional Information• Primary duties include conducting comprehensive PT and OASIS assessments, developing individualized plans of care, and delivering skilled therapy treatments in patients' homes,• Acts as Care Manager in coordinating care across disciplines, including supervision of PTAs, PT students, and home care aides,• Responsible for accurate and timely completion of all agency, state, and federal documentation including HHABNs and Medicare Notices of Non-Coverage,• Collaborates with Quality Improvement staff to ensure accuracy of clinical notes and OASIS documents,• Participates in peer review audits and in-service training for rehabilitation, nursing, and community groups,
