Individual should live in, Berks or Lebanon County, PA. This is full time employment opportunity for a Service Coordinator; Monday thru Friday 8am-4:30pm or 7:30 am to 4:00 pm. Position is 75% work from home and 25% travel to participant homes (located within Berks, Lebanon or Lancaster Counties), no on call. Persons must be self-motivated, have reliable transportation, reliable internet and the willingness to travel to participants homes. Familiarity with PA Medicaid is preferred, but not mandatory. Experience in related field or Bachelor's degree in a Social Science.
