Job Description

The Field Nurse will provide training and supervision to Unlicensed Assistive Personnel (UAP) under Registered Nurse Delegation (RND) including but not limited to training on nursing tasks, medication administration, taking vital signs, seizure precautions and other tasks directed by the RN and individual's physician. The nurse will ensure individual's orders are current and accessible to staff/caretakers. The nurse will assist with making referrals and ensuring the individuals meet their plan of care as directed by their physician and the Interdisciplinary team. The nurse will work in partnership with the RND to ensure safe and effective care plans and delegation. The nurse will work closely with accounting to ensure most effective billing and on ways to increase efficiency. Services occur within the office, assisted living communities, care homes, day programs and other long-term care communities. Applicant must have graduated from an accredited nursing program and be licensed by the state of Texas.
Field Nurse Job Duties:
assessing delegated nursing tasks to an unlicensed service provider and supervising the performance of those tasks in accordance with state law and rules;
monthly and prn supervision visits including assessment of UAP performance and client response to delegated tasks;
monitoring UAP performance of nursing tasks;
monitoring health risks, data, and information, including ensuring that an UAP is performing only those nursing tasks identified from a nursing assessment;
CRA communication and inclusivity in care planning;
care coordination with participating providers (hospice/home health) to ensure safe and effective care.
performing health care procedures ordered or prescribed by a physician or medical practitioner and required by standards of professional practice or law to be performed by an RN or LVN;
teaching an unlicensed service provider about the specific health needs of an individual;
skills training and remediation of UAP as necessary
ESSENTIAL KNOWLEDGE, SKILLS, AND ABILITIES:
Applicant must have graduated from an accredited nursing program and
licensed by the state of Texas
preferred 2 years of experience in community care
preferred experience teaching and supervising nursing tasks
