Social Services Director
The Social Services Director is responsible for supporting residents as well as their family members throughout their time at Parkview. Under the direction of the Administrator, you will be responsible for admissions process as well as a acting as a resource in the area of counseling, family dynamics/interactions. Market the facility to referral sources.Deal with payor sources like Medicare, Medicaid and insurance providers. Conducts psychosocial history to determine pre-injury issues which need to be addressed.Establishes and carries out individualized treatment plan upon completion of psychosocial history if applicable.Provides counseling to clients, families and/or significant others.Schedules and participates in team rounds and family conferences.Maintains records and prepare reports according to facility procedures.Participates in and documents internal and external professional educational activities to enhance personal and professional growth.Performs public relations task as directed by supervisor.Coordinates resident's schedule with physical therapy, occupational therapy, therapeutic recreation, nursing service, speech and language pathology, behavioral specialist and other disciplines as needed.Is an integral part of the interdisciplinary team.Assists in patient discharge planning.Conducts post discharge follow-up as needed.Any other tasks as assigned by the immediate supervisor.SPECIFICATIONS Formal Education/Formal Training: Must hold a Social Services Designee Certification or be able to obtain within 90 days of hire.Job Related Experience: Minimum of two years experience in a health care setting, preferably and Skilled Nursing Facility (SNF)
