RN Case Manager, Per Diem

International Quality Homecare of WI is seeking a per diem RN Case Manager for our Hayward, WI office and the surrounding areas of Rice Lake, Barron, etc. Per Diem RN - $40-$50 Skilled Nurse visits; $35/visit for PCA Supervisory Visits, plus travel time and mileage Basic Qualifications: The RN Case Manager position requires a minimum of an Associate degree in Nursing (RN) with at least 2 years of experience in home health or Nursing home. The Nurse must be licensed to work in the state of practice. The Nurse must have a valid driver’s license and a good driving record. Other Qualifications: The position requires a clear understanding of nursing in home care settings. The ability to assess the needs of the clients and work with individuals within and outside the company to meet the needs of the client. The position requires a demonstrated ability in strong analytical, problem-solving, communication, and leadership skills. Computer proficiency and the ability to learn different computer programs are highly required. Supervisory experience is preferred but not required. Job Description: The Nurse Case Manager provides nursing care to clients and works within and outside the company to ensure that their needs are met. Responsibilities: Prepares OASIS and other medical documents. Provides nursing care to the clients. Field Supervision of field staff involves communicating concerns regarding their performance to the Operations Coordinator for performance management and any necessary disciplinary action. Resolves Nursing and Administrative issues. Works with Caseworkers, Counties, Hospitals, and others that are involved in the case. Represents the interest of the company, client, and field nurses.
