This position reviews referrals, verifies eligibility, coordinates with referral sources, physicians, hospitals, and insurance providers, and ensures all required documentation is obtained prior to admission. The Intake Coordinator serves as a liaison between patients, families, clinical staff, and referral partners to facilitate timely and compliant patient admissions.Essential Duties And ResponsibilitiesReceive, review, and process skilled nursing referrals from hospitals, physicians, case managers, and other referral sources.Review patient clinical records to determine eligibility and appropriateness for skilled nursing services.Coordinate with physicians and clinical staff to obtain and verify admission orders and required documentation.Verify insurance coverage, benefits, authorizations, and payer requirements.Enter and maintain accurate patient information in the Electronic Medical Record (EMR) system.Schedule admissions and coordinate with nursing staff to ensure timely start of care.Communicate admission status and updates to referral sources, patients, and family members.Ensure compliance with Medicare, Medicaid, commercial insurance, and regulatory requirements.Track referral and admission metrics and maintain intake logs.Follow up on pending referrals and outstanding documentation.Collaborate with billing, authorization, and clinical departments to ensure smooth patient onboarding.Maintain confidentiality of patient information in accordance with HIPAA regulations.Assist with audits, quality assurance activities, and accreditation requirements as needed.QualificationsHigh school diploma or equivalent required; Associate's or Bachelor's degree in Healthcare Administration, Nursing, or related field preferred.Minimum 1–3 years of experience in healthcare intake, admissions, care coordination, home health, hospice, or skilled nursing preferred.Knowledge of Medicare, Medicaid, managed care, and insurance authorization processes.Experience with EMR/EHR systems and healthcare documentation.Understanding of skilled nursing services, clinical terminology, and patient care requirements.Strong organizational and multitasking skills.Excellent verbal and written communication skills.Proficient in Microsoft Office Suite (Word, Excel, Outlook).Skills and CompetenciesClinical record review and assessment supportInsurance verification and authorization managementAttention to detail and accuracyTime management and prioritizationCustomer service and patient relationsProblem-solving and critical thinkingTeam collaboration and coordinationKnowledge of healthcare regulations and compliance standardsPhysical RequirementsProlonged periods of sitting and computer work.Ability to communicate effectively via telephone, email, and in person.Ability to occasionally lift up to 15 pounds.Work EnvironmentOffice-based healthcare setting.Frequent interaction with referral sources, healthcare providers, patients, and family members.May require occasional overtime to meet admission and intake deadlines.Preferred Experience: Home Health, Hospice, Skilled Nursing Facility (SNF), Prior Authorization, Care Coordination, Intake/Admissions, Case Management, or Healthcare Administration.
