The Case Manager maintains a flexible, organized, and efficient work schedule and is subject to work evening and weekends (as necessary) and be on-call on a rotating schedule to facilitate program services.\n\nEssential Functions:\n\nConduct initial intake with client and family/guardian.\nEnsure the timely completion of all Intake paperwork, Assessments, and Service Plans in accordance with Southwest Key Programs (SWK) policy.\nEnsure all documentation is completed and entered into SWK database within 48 hours of completion of the service.\nMaintain physical and web-based client files.\nEstablish and maintain therapeutic relationships with clients and families.\nProvide crisis intervention for clients and their families as needed.\nProvide home visits with clients and families per the program service schedule.\nEnsure clients are offered all components or services offered at the program.\nProvide advocacy specified by Southwest Key Programs and contract guidelines.\nLink families to natural and professional support services as needed.\nFollow-up on all evaluations and assessments of the child’s progress in accepted interventions.\nMaintain connection with school, and other community partners as needed.\n\nOther Functions:\n\nAttend and participate in all required professional trainings and workshops.\nMaintain open and consistent communication with Program Director or designee, clinician, and other team members.\nTransport clients to program activities, school hearings or other treatment meetings as needed.\nAble to react to change productively and handle other essential tasks as assigned.\n\nQualifications and Requirements:\n\nA combination of one of the following:\nBachelor’s degree in Behavioral Sciences, Human Services, Social Service or related field with two (2) years’ experience working with at risk youth and their families; may include part time, volunteer or internship experience.\nMust be computer literate with working knowledge of Microsoft Windows (Microsoft Word, Excel) variety of online database systems.\nRequired to work a flexible schedule to facilitate program services.\nWorks well independently and as an effective team member.\nSuccessfully pass pre-employment (post offer) background check.\nMust possess a valid state driver’s license, registration and maintain state mandated car insurance.\nMust be at least 21 years of age at the time of hire.\nPossess good verbal and writing skills.\n\nPhysical Demands:\nMust be able to obtain First Aid Certification (CPR). Must be able to verify client’s whereabouts by driving, sitting, standing and walking for extended periods of time. Daily use of various office equipment. Bending, stooping, climbing up stairs and lifting up to 15 lbs. required to complete daily tasks.\n\nWork Environment:\nThis is a program which requires daily contact at various client locations. Work shifts are subject to change. A busy environment with many unscheduled interruptions. Noise levels vary from mild to moderate. This position may require extended periods of time in an automobile.
